Change of Online Prescription Service   If you use the online prescription service, you need to come into the Surgery to register for a new password - please bring along photo ID to do this. 


WE ARE A YELLOW FEVER CENTRE! You do not have to be registered with us to access this service.


Travel Clinic
We can provide a full Travel Health Service to ensure your health is protected when you go abroad. 


Out of Hours
We are covered by the Frimley Primary Care Services Limited, based at Frimley Park Hospital and accessed by dialling 111.


Prescriptions
There are several ways to request or collect your prescriptions. Please check under the 'Prescriptions' page.


Test Results
We are extremely busy with telephone calls for the first 2 hours of the morning - to help us please do not phone for results until after 10.00 a.m.


Home Visits
Visits should be requested, where possible, before 10.00 a.m.  They are made only at the doctor's discretion.  Please ensure you leave a contact number and address when phoning, as the doctor is likely to phone you before making the home visit.

Some services provided are not covered under our contract with the NHS and the work is therefore carried out by the GPs after they have seen their patients and dealt with the paperwork associated with their appointments.  This means that all private work, such as medical reports for insurance companies and letters to authorities can take a minimum of 10 days to process.  Please also remember that the majority of non-NHS work attracts a charge, so please ask what the fee is when handing in non-NHS work.  Examples include the following:

  • Medicals for pre-employment, sports and driving requirements (HGV, taxi, occupational, etc.)
  • Adoption medicals/reports
  • Employer medical reports
  • Insurance claim forms (accident, travel, private health, etc.) 
  • Passport signing
  • Verification of identity
  • Private prescriptions
  • Prescriptions for taking medication abroad
  • Private sick notes
  • Declarations of health (gym membership, parachute jumps, etc)
  • Vaccination certificates
  • Power of attorney assessments/reports
  • Letters including ‘To Whom It May Concern’ and ‘Fitness to …’ letters.  Letters and reports for insurance companies and solicitors are always chargeable.  Please check with the company concerned first to see who has to pay for the work required.
  • Photocopying and printing of medical records and summaries

The fees charged are based on the British Medical Association (BMA) suggested scales and a full printed list of the charges applicable can be found on the wall by the reception desk.  If you have any queries, our Reception Staff or Medical Insurance Clerk will be happy to advise you about them along with appointment availability, if required.
