IMPORTANT INFORMATION – How we use your data

There have been recent articles in the press and online about the NHS sharing your data and having the option to opt-out of this.

We have debated whether or not to publicise this as there is no requirement for us to do so but we also strongly believe in giving our patients a choice so please read the following information to help you make your decision.

The data held in the GP medical records of patients is used every day to support health and care planning, and research in England to help find better treatments and improve patient outcomes for everyone.

NHS Digital has developed a new way to collect this data, called the General Practice Data for Planning and Research data collection (GPDPR).

There are two types of opting out to consider –

Type 1 stops any data going from the practise to NHS Digital.  If you do not want your patient data to be shared outside of your GP practice for purposes except your own care, you can register to opt-out with us by completing this form and either email it to the surgery at or pop it in the post to us.

National Data Opt Out (Type 2) allows data to go to NHS Digital and for further information on how they will use this data and to opt out, go to
